Provider Data Collection

SOR-funded providers are required to complete a SUPRT-A assessment for every SOR-funded client at the following assessment points: baseline, reassessment, annual, and closeout. Once a SUPRT-A baseline is submitted, providers will use the provider dashboard to submit all subsequent data. The provider dashboard will display which clients are currently due for each type of assessment.
