Ready to Complete a GPRA Intake?
SOR-funded Opioid Treatment Providers are required to complete a GRPA Intake following the three steps outlined below. All GPRA data submissions are submitted online and should be completed within three days of the GPRA clients’ induction to treatment. Contact The Evaluation Center for GPRA Intake Training or any questions related to the SOR GPRA Provider Requirements.
Gain Client Consent
Review the consent form with SOR clients prior to beginning the GPRA data collection. Signed consent forms should be stored on-site. Clients that refuse to participate may receive SOR-funded treatment (see the FAQs for more information). Click to view and download the consent form.
Complete Client Contact Form
Submit client contact information online. The contact form is required for all GPRA clients. Updated contact information may also be submitted for existing GPRA clients. Click to complete a contact form.
You must create a GPRA Client ID to submit a client contact form.
Administer GPRA Survey
Complete the GPRA intake survey for all GPRA clients. GPRA data is submitted into the online SAMHSA Performance Accountability and Reporting System (SPARS). Once you submit a contact form, you will be redirected to the SPARS portal. If you need a SPARS account, submit a request below.
